Every time you donate blood, platelets or red blood cells at a Lifeblood donor center or mobile drive, you earn special benefits and points that you can redeem for gifts from the Club Lifeblood online store.


Benefits and points start to accrue with your first donation, but to claim them you must be a current Club Lifeblood member.


Enrolling is easy. Just fill out the form in this brochure and give it to a Lifeblood representative. You’ll receive confirmation of enrollment either by e-mail or regular mail, and you’ll be able to check your points online or by telephone throughout the year.


Benefits and Recognition:


Club Lifeblood benefits depend upon how many times per year you donate. Benefits include health screenings and tests, as well as credits for the Blood Usage Benefit program that covers you, your spouse and your dependent children should any of you need blood or blood products during hospitalization.

 


Blood Usage Benefit


For every 2 donations (whole blood or platelet) or 1 donation of double red cells a donor, his/her spouse or tax-deductible dependent (as defined by the IRS) is eligible for up to $100 voucher to be used toward blood and blood products up to $1,200 per year.  To print a claim form, Click here.

How Does It Work?

  1. An individual covered by the plan (see above) uses blood in the hospital.
  2. That person is released from the hospital and receives a bill and explanation of benefits on what insurance has covered.
  3. Person can apply for coverage of any blood or blood product fees not paid by insurance, excluding transfusion related costs (e.g. cross-matching or blood typing), administrative costs or other treatment related costs.
  4. The donor/patient has six months from the date of release from care of the hospital or facility to file a claim for coverage.
  5. The donor can download or print the claim form on ClubLifeblood website or www.lifeblood.org or contact Lifeblood by phone or e-mail to have a form mailed or faxed.
  6. The donor completes the claim form submits the form to Lifeblood: " through website " through mail " by fax " by e-mail along with their itemized bill showing blood fees remaining on the bill reimbursed by insurance and an explanation of benefits (EOB) from the insurance company showing the item was not paid by insurance. The itemized hospital bill must include the patient's name, mailing address, hospital, dates of transfusion, procedure description and number of units received. A summary bill is not adequate.
  7. Lifeblood administrator will verify blood usage and insurance payments applied to blood and blood product fees.
  8. Lifeblood Administrator submits summary to Lifeblood accounting department to cut check to individual and hospital, jointly.
 
Rewards:
Both donors and the representatives who sponsor blood drives earn points through Club Lifeblood. Earn extra points by donating or hosting a drive during a Holiday week, by bringing a new donor to a Lifeblood center or by exceeding your annual goal.
